If you have a communication need and would like to receive information in a different format to help you to understand please inform us.  Such formats could include large text, easy-read documents, or you may require an interpreter for appointments.

To inform us please ask a receptionist, or click here and complete the comments or suggestions online form.  Once we have this information, we will record it in your records so we can take steps to ensure you receive the information you can access and understand and also receive any communication support you require.

Did you know that we have a smoking cessation advisor in the surgery, please call in to make an appointment if you would like help to quit smoking.

For a healthy lifestyle good diet and exercise are beneficial.  Please ask one of our Health Care Assistants for advice and support.

Are you or your partner a carer?  Help is available, please ask our reception staff for a carers pack.  There is also a local group who can offer support and advice, they are called Carers Together, St Mary's Centre, 82-90 Corporation Road, Middlesbrough TS1 2RW tel 01642 488977
