Everyone needs a bit of help from time to time.

Social Prescribing is a way of helping you to deal with the things in your life that you feel need more focus and always keeping you in control of your support. Our service can help you access a wide range of activities and support suited to your needs and interests that can help turn things around.

We understand that our finances, family life, physical health and lots of other factors can have an impact on our lives.

Link Workers help you work out what can make a difference and connect you to practical and emotional community support in Teesside, through voluntary and charitable organisations.

Your journey could include volunteering, training opportunities, arts activities, cooking, sports, exercise and so much more. We support people to improve their lifestyle one step at a time, addressing any challenges you may face along the way, whilst also exploring self care; this could mean getting out into the fresh air in Stewart’s Park, learning a new hobby (we’ve even been known to count eels for a conservation project on the Tees!), or participating in sessions at a local football foundation. If the great outdoors isn’t for you, there are a wide range of creative and virtual activities that you can be involved with too.

Our team has worked hard to build relationships with individuals and organisations throughout Teesside, to keep our finger on the pulse and really get to know first-hand what experiences and opportunities are out there. Our team work alongside you over time to explore what works for you, to find out what is important to you and produce a personalised plan with you. This will help you to reach your goals, to stay healthy and to maintain your wellbeing.

We work in both Linthorpe and North Ormesby sites, as well as other practices in Holgate PCN. Feel free to ask about us through any member of staff at the GP practice, if you feel like you may benefit from our help or even just want to find out a little bit more.
